You may click the link below to print our New Patient Forms prior to your appointment.

Please complete each page and bring them with you to your scheduled appointment along with your photo ID and insurance card.

Please remember to bring these forms with you.

We ask you arrive at our office 15 minutes prior to your appointment.

To better serve you we would also ask that you bring to your appointment:
Any previous medical records
X-Rays, CT or MRI Scans
Sleep study results
Blood Work or Lab Results
Any information that would be helpful to assist the doctor with your care.

If your insurance requires a REFERRAL, please contact your Primary Care Physician PRIOR to your appointment so they may fax it to our office at 972-432-0552. Most doctors require a 48 to 72 hour notice to issue a referral.
